Self-Pay Patients

Acknowledgment of Financial Responsibility

By receiving services at LenoyMED Medical Spa, you acknowledge full financial responsibility for all services rendered. Payment is due in full at the time of service or purchase unless a written payment arrangement has been agreed upon in advance.

Most aesthetic, wellness, and elective procedures are not covered by health insurance and are provided on a self-pay basis. For these services, you are voluntarily electing to forgo any insurance coverage or benefits you may have, and LenoyMED will not submit any claim to a health insurance plan on your behalf. You agree not to bill or seek reimbursement from any insurance plan for these services, and you understand that amounts you pay will not be applied to any insurance deductible, co-payment, or out-of-pocket maximum. You accept full financial responsibility for all charges, regardless of whether any third party might otherwise have provided coverage.

Payment Methods Accepted

We accept:

• Credit and Debit Cards
• Approved Digital/Online Payments
• Financing through approved third-party providers
• Cash (if permitted at specific locations)

A deposit may be required to reserve certain appointments or to purchase certain packages, as disclosed at the time of booking. LenoyMED reserves the right to modify accepted payment methods at any time.

Pricing, Fees & Card Surcharge

• Pricing is presented as follows: the Total Regular Price, less any applicable Discount, equals the Selling Price; applicable card-processing fees are then added to arrive at the Final Selling Price.
• Where payment is made by credit or debit card, a card-processing surcharge may apply. Any such surcharge will be disclosed to you before the transaction is completed.
• Prices are subject to change; the price in effect at the time of purchase applies to that purchase.
• Where a price is given as an estimate, the final amount is based on the services actually performed. You will receive an itemized receipt of the services and their cost.

Prepaid Services & Packages

• All prepaid services must be paid in full at the time of purchase unless financed.
• Prepaid treatments, packages, and services are non-refundable, non-transferable, and non-redeemable for cash.
• Prepaid services and packages are valid for twelve (12) months from the date of purchase, unless a different period is stated in writing at the time of purchase.
• Packages cannot be paused, frozen, reactivated, or extended for any reason — including medical leave, pregnancy, travel, relocation, or personal circumstances.
• All services within a package must be used by the same client and are not transferable.
• Any services left unused at the end of the validity period, or upon termination, cancellation, or expiration of the package, expire and are forfeited in full, with no carryover, credit, or refund, except as otherwise required by law.

• LenoyMED maintains records of all appointments and session usage. You are responsible for verifying your balance at each visit; any discrepancy must be raised before leaving the premises.

   

Prepaid Services & Packages

Payment is for the performance of the selected service or the provision of the product, not for any particular result. Individual outcomes vary, and no specific result is guaranteed.

Insurance Patients

Insurance Coverage

LenoyMED may provide limited medically necessary services that qualify for insurance reimbursement. Insurance coverage is determined solely by your insurance carrier.

You are responsible for:

• Understanding your policy benefits, exclusions, limitations, and authorization requirements.
• Obtaining any required referrals or prior authorizations before treatment.
• Responding promptly to insurance requests for additional information.

LenoyMED will attempt to verify that your coverage is valid at the time of your visit; however, if your coverage is not in effect at that time, you will be responsible for payment in full. Any benefit information we obtain is a quote of benefits and not a guarantee of payment. Final determination of payment is made by your insurance carrier when the claim is processed.

Co-Payments, Co-Insurance & Deductibles

• Co-payments are due at the time of service.
• Deductibles and co-insurance amounts are the patient's or guarantor's responsibility.

• If insurance denies a claim for any reason — including a determination that a service was not medically necessary, was cosmetic, or was otherwise excluded — the balance becomes the patient's responsibility.

   

Non-Covered & Cosmetic Services

Many services offered by LenoyMED are cosmetic, aesthetic, or elective in nature and are generally not covered by health insurance; LenoyMED will not submit claims to insurance for such services. If you elect a service that is not covered by your insurance, you will be required to sign and abide by the LenoyMED Financial Responsibility — Self-Pay Consent for that service, and full payment is due at the time of service or purchase.

Referrals & Authorizations

If your plan requires a referral or prior authorization, it is your responsibility to obtain it before your appointment. If you choose to be seen without the required referral or authorization, payment is due at the time of visit. If a referral to a specialist is required, please contact your primary care physician at least one (1) business day before your appointment.

Assignment of Benefits & Release of Information

You assign to LenoyMED all insurance benefits otherwise payable to you for services rendered and authorize your insurance carrier to issue payment directly to LenoyMED. You authorize LenoyMED to release any medical and billing information necessary to process insurance claims, secure payment, and respond to your insurance carrier's requests, consistent with applicable privacy laws including HIPAA.

Out-of-Network & Balance Billing

If LenoyMED is not in-network with your insurance carrier, your out-of-pocket costs may be significantly higher, and your carrier may reimburse only a portion of the amount billed. You are responsible for any remaining balance, subject to applicable law. Where required by law, LenoyMED will provide notice of out-of-network status and any applicable consumer protections before rendering non-emergency services.

Insurance Payments Issued to Patient

If reimbursement is sent directly to you, you are required to forward payment to LenoyMED promptly, along with the Explanation of Benefits (EOB).

Cancellation & No-Show Policy

• Purchase cancellation. You may cancel a purchase within twenty-four (24) hours of the time of purchase, subject to the Non-Refund Policy below.
• Appointment cancellation. Appointments must be canceled or rescheduled at least 24 hours prior to the scheduled time.
• Appointments cancelled with less than 24 hours' notice, or missed without notice, may be subject to a fee of $50.00 or forfeiture of the booked service. For insurance-billed visits, this fee is the patient's personal responsibility and is not billable to insurance.
• Repeated missed appointments may require prepayment for future bookings.

Returned Payments / Chargebacks

• A fee of $50.00 will be assessed for any check returned for non-sufficient funds or failed ACH transaction, in addition to any fees charged by your bank.
• Disputed charges or unauthorized chargebacks will be investigated and may result in suspension of services.
• Client agrees to reimburse LenoyMED for any collection costs, legal fees, or administrative expenses incurred in recovering unpaid balances.

Payment Policy

General Payment Terms

• All balances are due at the time of service.
• LenoyMED may store payment information securely, in accordance with PCI-DSS standards, for recurring or future transactions.
• By providing your card information, you authorize LenoyMED to retain your card on file and charge it — without a separate signature per transaction — for the products and services you purchase; any agreed payment-plan or membership installments; outstanding balances; and any applicable cancellation, no-show, or late fees.
• For any recurring or installment charge, the amount (or how a variable amount is calculated), frequency, and duration are disclosed when the plan is set up, with advance notice of any change.
• LenoyMED may re-attempt a failed charge; you remain responsible for the balance plus any applicable fee, and services may pause until payment clears.
• This authorization remains in effect until revoked in writing to the LenoyMED front desk, effective after a reasonable time to act (at least 3 business days); revocation does not affect charges already incurred or services already scheduled.

• Failure to maintain valid payment information may result in suspension of services.

   

Financing

Financing may be available through third-party providers. All financing agreements are between the client and the financing company.

LenoyMED:

• Does not control financing approval decisions.
• Is not responsible for financing terms, interest rates, or repayment schedules.
• Is entitled to full payment regardless of the client's financing status.

Past Due Accounts

LenoyMED reserves the right to:

• Suspend services
• Revoke membership privileges
• Refer accounts to collections
• Assess late fees and/or interest where permitted by law

Balances not paid within thirty (30) days of statement may be subject to additional collection efforts and applicable fees. Client is responsible for all costs of collection, including reasonable attorney's fees.

Non-Refund Policy

Unless explicitly required by law or stated in writing, all sales are final. The following are non-refundable, non-returnable, and non-transferable:

• All services rendered
• Membership dues
• Promotional purchases
• Initiation fees, deposits, and prepaid services
• Complimentary or promotional services (which have no cash value)

If, in LenoyMED's sole and absolute discretion, a refund or store credit is granted, the decision is final and binding. Where a refund or store credit is granted, any services already received or consumed will first be deducted and re-billed at their regular (non-discounted) price, and only the remaining balance, if any, will be refunded or issued as credit. Store credit, where issued, is non-transferable and subject to the same validity period as prepaid services.

E-Commerce

Online Purchases

• Products and services purchased online are subject to this Payment Policy and Terms & Conditions.
• Orders must be paid in full prior to processing.
• Pricing is subject to change without notice.
• LenoyMED reserves the right to cancel orders due to pricing errors, suspected fraud, or inventory issues.

Shipping & Returns (Retail Products Only)

• Returns are accepted only for unopened, unused retail products within the stated return window.
• Services, gift cards, memberships, and digital purchases are non-returnable.
• Shipping fees are non-refundable unless required by law.

Fraud Prevention

LenoyMED reserves the right to:

• Verify billing information
• Request identification

• Refuse service in cases of suspected fraud

   

Promotions

Buy-X-Get-Y & Discount Offers

• Free or discounted services must be redeemed within the promotional validity period.
• A discount applies only to the specific service and quantity purchased and cannot be exchanged for cash or applied toward other services or products.
• Unused promotional services expire and hold no cash value.

• Promotional services are non-transferable.

   

Refund Restrictions

All promotional purchases are considered final sale unless otherwise required by law. In the event a partial return is permitted, the used portion will be charged at the regular (non-discounted) price, and the remaining balance will be refunded.

Promotional Offers

• Promotions are valid only during the advertised period.
• Promotions cannot be combined with any other promotion, offer, membership benefit, or loyalty credit unless explicitly stated in writing.

• Promotional pricing requires full compliance with stated terms (e.g., minimum unit purchases) and payment in full at the time of purchase.

   

Membership, Subscription & Rewards

Membership Overview

• Memberships are personal, non-transferable, non-assignable, and non-shareable — only the named member may use the membership.
• By enrolling, you agree to all membership terms, including the automatic renewal provisions below.

• Each treatment received through a membership remains subject to a separate service-specific consent form, medical eligibility, and provider discretion.

   

Minimum Commitment & Auto-Renewal

• Membership requires a minimum commitment of six (6) months (the "Initial Period").
• After the Initial Period, membership automatically renews for successive 6-month terms unless cancelled in accordance with the Cancellation Rights below.
• Cancellation is not permitted during the Initial Period, except for the limited rights set out below; early termination may result in charging the remaining balance.

• To avoid automatic renewal, written notice of cancellation must be submitted to the LenoyMED front desk at least thirty (30) days before the end of the current term. Verbal requests will not be accepted.

   

Cooling-Off Right

You may cancel within three (3) business days of signing for a full refund, provided no membership benefits or services have been used.

Membership Dues

• Dues for the selected plan are billed automatically to the card on file on the same day each month.
• Dues are owed regardless of whether you use any service; failure to use services does not relieve payment obligations.
• Unused monthly benefits — including any complimentary service — do not roll over or accumulate.

• If dues are not paid when due, LenoyMED may suspend benefits and pursue collection of amounts owed; cancellation does not relieve any past-due balance.

   

Cancellation Rights

In addition to the Cooling-Off Right and the 30-day renewal notice above, members (or their estate) may cancel for a pro-rated refund of unused, prepaid amounts if:

• The member dies or becomes permanently disabled (documentation may be required); or
• LenoyMED permanently closes or relocates more than 10 miles.

Refunds, where applicable, will be calculated on a pro-rated basis as permitted by law. Except as expressly stated, no refunds are issued, and upon cancellation any unused benefits or credits terminate.

Revocation of Membership

LenoyMED reserves the right to suspend or revoke membership for:

• Non-payment
• Policy violations
• Disruptive or inappropriate conduct

Where a membership is suspended or revoked, any prepaid, unused amounts are refunded less amounts owed. Revocation does not eliminate outstanding financial obligations.

Rewards Programs

Reward points:

• Has no cash value
• Are non-transferable
• May expire
• May be modified or discontinued at LenoyMED's discretion
