Service

  1. Self-Pay Patients
     

    1. Acknowledgment of Financial Responsibility
       

      By receiving services at LenoyMED Medical Spa, you acknowledge full financial responsibility for all services rendered. Payment is due at the time of service unless prior written arrangements have been made.

      All aesthetic, wellness, and elective procedures are considered self-pay unless otherwise verified as insurance-covered services.

    2. Payment Methods Accepted
       

      We accept:

      1. Credit and Debit Cards
      2. Approved Digital/Online Payments
      3. Financing through approved third-party providers
      4. Cash (if permitted at specific locations)
      5. LenoyMED reserves the right to modify accepted payment methods at any time.
    3. Prepaid Services & Packages
       

      1. All prepaid services must be paid in full at the time of purchase unless financed.
      2. Prepaid treatments, packages, and services are non-refundable, non-transferable, and non-redeemable for cash.
      3. Services must be used within the validity period stated at purchase.
      4. Unused services expire upon termination, cancellation, or expiration of the package.
  2. Insurance Patients
     

    1. Insurance Coverage
       

      LenoyMED may provide limited medically necessary services that qualify for insurance reimbursement. Insurance coverage is determined solely by your insurance carrier.

      You are responsible for:

      1. Understanding your policy benefits, exclusions, and authorization requirements.
      2. Obtaining any required referrals prior to treatment.
      3. Responding promptly to insurance requests for additional information.
      4. Verification of insurance eligibility does not guarantee payment.
    2. Co-Payments, Co-Insurance & Deductibles
       

      1. Co-payments are due at the time of service.
      2. Deductibles and co-insurance amounts are the patient’s responsibility.
      3. If insurance denies a claim for any reason, the balance becomes the patient’s responsibility.
      4. Insurance Changes
        Failure to notify LenoyMED of insurance changes may result in claim denial and full financial liability.
      5. Insurance Payments Issued to Patient
        If reimbursement is sent directly to you, you are required to forward payment to LenoyMED immediately along with the Explanation of Benefits (EOB).
  3. Cancellation & No-Show Policy
     

    1. Appointments must be canceled or rescheduled at least 24 hours prior to the scheduled time.
    2. Late cancellations or no-shows may incur a cancellation fee.
    3. Repeated missed appointments may require prepayment for future bookings.
  4. Returned Payments / Chargebacks

    1. A processing fee may be charged for returned checks or failed ACH transactions.
    2. Disputed charges or unauthorized chargebacks will be investigated and may result in suspension of services.
    3. Client agrees to reimburse LenoyMED for any collection costs, legal fees, or administrative expenses incurred in recovering unpaid balances.

Payment Policy

  1. General Payment Terms

    All balances are due at the time of service.
    LenoyMED may store payment information securely for recurring or future transactions.
    Client authorizes LenoyMED to charge the payment method on file for outstanding balances, late fees, or installment plans.
    Failure to maintain valid payment information may result in suspension of services.
     

  2. Financing

    Financing may be available through third-party providers.
    All financing agreements are between the client and the financing company.
    LenoyMED:

    1. Does not control financing approval decisions.
    2. Is not responsible for financing terms, interest rates, or repayment schedules.
    3. Is entitled to full payment regardless of client’s financing status.
  3. Past Due Accounts

    LenoyMED reserves the right to:

    1. Suspend services
    2. Revoke membership privileges
    3. Refer accounts to collections
    4. Assess late fees and/or interest where permitted by law
    5. Client is responsible for all costs of collection, including reasonable attorney’s fees.
       
  4. Non-Refund Policy

    Unless explicitly required by law or stated in writing:

    1. All services rendered are non-refundable.
    2. Membership dues are non-refundable.
    3. Promotional purchases are non-refundable.
    4. Initiation fees, deposits, and prepaid services are non-refundable.
    5. Complimentary or promotional services have no cash value.
 
E-Commerce
  1. Online Purchases
    Products and services purchased online are subject to this Financial Policy and Terms & Conditions.
    Orders must be paid in full prior to processing.
    Pricing is subject to change without notice.
    LenoyMED reserves the right to cancel orders due to pricing errors, suspected fraud, or inventory issues.

     

  2. Shipping & Returns (Retail Products Only)
     

    Returns are accepted only for unopened, unused retail products within the stated return window.
    Services, gift cards, memberships, and digital purchases are non-returnable.
    Shipping fees are non-refundable unless required by law.

  3. Fraud Prevention

    LenoyMED reserves the right to:

    1. Verify billing information
    2. Request identification
    3. Refuse service in cases of suspected fraud

      Promo

    Buy-X-Get-Y & Discount Offers

    Free or discounted services must be redeemed within the promotional validity period.
    Unused promotional services expire and hold no cash value.
    Promotional services are non-transferable.

     

    Refund Restrictions

    All promotional purchases are considered final sale unless otherwise required by law. In the event a partial return is permitted, the used portion will be charged at the regular price and the remaining balance will be refunded.

     

    Promotional Offers

    Promotions are valid only during the advertised period.
    Promotions cannot be combined unless explicitly stated.
    Promotional pricing requires full compliance with stated terms (e.g., minimum unit purchases).

 
Membership, Subscription, Rewards
 
  1. Membership Overview

    Memberships are personal, non-transferable, and non-assignable.
    By enrolling, you agree to all membership terms, including automatic renewal provisions.

  2. Minimum Commitment & Auto-Renewal

    Memberships may include an Initial Commitment Period.
    Membership automatically renews unless canceled in accordance with written notice requirements.
    Notice of cancellation must be submitted in writing at least thirty (30) days prior to renewal.

  3. Membership Dues

    Dues are billed monthly or annually as selected.
    Dues are not based on usage.
    Failure to use services does not relieve payment obligations.

     
  4. Cancellation Rights

    Members may cancel:

    1. Within three (3) business days of enrollment for a full refund if no services have been used.
    2. In cases of death or permanent disability (documentation required).
    3. If LenoyMED permanently closes or relocates beyond a reasonable service radius.
    4. Refunds, if applicable, will be calculated on a prorated basis as permitted by law.
  5. Revocation of Membership

    LenoyMED reserves the right to suspend or revoke membership for:
    Non-payment
    Policy violations
    Disruptive or inappropriate conduct
    Revocation does not eliminate outstanding financial obligations.
     
  6. Rewards Programs
    Reward points:
    1. Have no cash value
    2. Are non-transferable
    3. May expire
    4. May be modified or discontinued at LenoyMED’s discretion
